Please have the    following information available at the time of your appointment:

 For all tax returns:

• Prior year tax return if available
• A picture id for taxpayer and spouse
• Social security cards for taxpayer, spouse, and dependents
• Taxpayer and spouse full names and dates of birth
• Dependents - full names and Dates of birth
• Dependent care expenses - need provider's name, address, tax payer identification #, and total amount paid for the year. Most daycares will provide this information.
• W-2's for all jobs worked during the 2008 tax year
• Information on any additional income not reported on a W-2

***Note-for Refund Anticipation Loans - a copy of both a drivers license and social security card for taxpayer and spouse, and social security cards for dependents, are required to apply for a "rapid" Refund Anticipation Loan.
